What Documents are required for Colombia Temporary Visa for Medical Treatment?
There are some general requirements when applying for Colombia temporary visa for medical treatment which are the following:
Visa Application Form:
Form DP-FO-67 “Application for a visa” completely filled out and signed by the foreign applicant, not by an intermediary.
Valid Passport:
Present a valid passport or travel document, in good condition and with a minimum of two (2) blank pages.
Passport Photocopy
A photocopy of the first page of your valid passport showing your biographical data. The photocopy can be either in color or black and white.
Proof of Legal Entry:
A photocopy of the page of your passport showing last stamp of entry or departure of Colombia.
Photocopy of Previously Issued Visa:
If you’ve had a previous Colombian visa such as a student visa, then a photocopy of this visa.
Medical Certificate:
A Certification issued by the medical doctor stating the need for the treatment and the time it requires, countersigned by the corresponding medical institution, as the case may be.
Financial Solvency Document:
A document certifying the financial solvency of the foreigner and of the person who guarantees the expenses of his/her stay in Colombia during the time required for the treatment, which must be coherent with its cost.
Photocopy of Return Tickets:
Photocopy or electronic information of the respective tickets for departure from Colombia.
Passport photo:
Passport style face photo with a white background, sized at 3 cm width X 4 cm height, maximum size of 300 kb jpg file for online application.
Proof of Health Insurance
Health policy with coverage in the national territory against all risks in case of accident, illness, maternity, disability, hospitalization, death or repatriation, for the time foreseen for their stay in the country.
If you are a primary visa holder and your beneficiary is seeking visa for medical treatment, then the following documents are required.
Document Proving Kinship:
A civil registration or equivalent document showing the relationship or kinship, duly legalized or with an apostille, as the case may be.
Financial Support Document:
A documents evidencing the economic dependence of the beneficiary with respect to the visa holder
Letter of Support:
A letter in which the visa holder assumes responsibility for the beneficiary’s stay in and departure from Colombia. In the case of minors such as children under the age of 18, the application, proxy or authorization must be signed by the parents and authenticated before a notary or Colombian consul.
Photocopy of Primary Visa Holder:
A photocopy of the holder’s valid visa.
Please note that when the request for a visa as a beneficiary is presented to the Visas office in Bogota, the foreign permanent partner of the visa holder must be present.
The primary visa holder is required to attend the respective judicial decision, minute of conciliation or public deed declaring the existence of the de facto marital union in accordance with Colombian regulations.
The date of issue of this deed cannot be more than three (3) months or 90 days prior to the day on which the request for a visa is presented.
